Why it works

The orange shadow is the argument. It makes the disease bigger than the child and present in the room without drawing anything medical, which is how the poster gets past the squeamishness that killed most syphilis campaigns of the period. The two headline registers do different jobs, the euphemism The Great Crippler is set in calm classical caps so it reads as a fact, while the word SYPHILIS is fractured stencil lettering that performs the damage. Naming the disease at all was the campaign's actual breakthrough, and the design commits to it by making that one word the largest thing on the sheet. No instruction, no clinic address, no statistic, the whole persuasion is carried by the picture and two lines.

Steal this when

Use this when the subject is taboo or abstract and naming it plainly is itself the move, and when the consequence can be embodied by a single figure. The oversized shadow device works whenever a threat needs to be shown as larger than its visible victim.
